- Short form consents revised. See Forms. Guide to the IRB section How do I obtain consent from non-English Subjects? revised. Schedule of IRB meetings for the second half of 2008 added. (12-May-2008)
- Minor revision to the modification form: The sentence "This form should not be used for a violation or deviation." was added to the instructions, first page. See Forms. (29-Apr-2008)
- Revisions to the renewal form: The title has been changed to "Request for Renewal of IRB Approval or Study Closure." On page 2, under "Action requested by PI" the second sub-title has been changed to "Closure of Study." Form "Application for IRB Approval of Human Subjects Research" has been updated to reflect changes in SSN policy, to correct some broken web links, and to revise the instructions on the cover sheet about use of Section C.
